Please submit this completed Proposal Form along with:
- Current CPR & First Aid Cards
- Any additional certifications you hold
- A current resume
- Proof of insurance (We will ask for an official Certificate of Insurance naming the Town of North Salem as additionally insured once your program has been approved.)
Submit your completed form and all required documents to recreation@northsalemny.org or mail/drop off to North Salem Recreation Dept, 270 Titicus Road, North Salem, NY 10560
