Birth Records

Who is eligible to get a copy of a birth certificate?

• The person named on the birth certificate.
• A parent of the person named on the birth certificate. The requesting parent's name must be on the birth certificate.
• A spouse, child or others, but only by order of a New York State court.

What identification needs to be submitted by the applicant?

One of the following forms of valid photo ID:

• Driver's license
• State-issued, non-driver photo ID card
• Passport
• U.S. Military issued photo ID

OR

Two of the following showing the applicant's name and address:

• Utility or telephone bill
• Letter from a government agency dated within the last six months

Please submit a copy of your U.S. passport in addition to the above ID if you are applying from a foreign country that requires a passport for travel.

Fee: $10 per certified copy.

Payment Options: Cash, Money Order, Certified Check or Personal Check made payable to Town of North Salem.

Death Records

Who is eligible to get a copy of a death certificate?

• The spouse, parent, child or sibling of the deceased
• Other people who have a:
  • documented lawful right or claim
  • documented medical need
  • New York State Court Order

If you are not the spouse, parent, child or sibling of the deceased, you must document a lawful right or claim. For example, you may need a death certificate to claim a benefit. You would need an official letter from the agency saying you need the death record to process the claim.

• Spouses
• Other people who have a:
  • documented judicial or other proper purpose
  • New York State Court Order

If you are not the spouse, you must document a judicial or other proper purpose. For example, you may need a marriage certificate to claim a benefit. You would need an official letter from the agency saying that you need the marriage record to process the claim.

Genealogical Records

All requests must be submitted in writing and include payment of the applicable statutory fee. Mail in the application form (form to be uploaded), include the fee as detailed on the form, with a check payable to Town of North Salem, 266 Titicus Road, North Salem, NY 10560.
